What Is a DEXA Scan?

Why the Scale and BMI Fall Short
A bathroom scale gives you one number: total weight. It cannot tell you whether a pound is muscle or fat, and it cannot tell you where your fat is stored. Two men can weigh exactly the same and have completely different metabolic health.
Body mass index has the same blind spot. BMI is only weight adjusted for height, so a muscular person can be labeled overweight while someone at a normal BMI can carry a risky amount of internal fat. Visceral fat behaves differently from the fat under your skin. It is metabolically active, functioning almost like an organ that releases inflammatory signals and free fatty acids directly toward the liver. Visceral fat is the strongest fat-related predictor of heart disease, and research shows it can track with abnormal blood pressure, triglycerides, and blood sugar even before those standard tests turn abnormal.
A useful benchmark: a visceral fat area above roughly 100 cm² is generally read as a marker of elevated cardiometabolic risk, and risk climbs further from there. Visceral fat also roughly doubles between the mid-forties and mid-sixties in men, which is one reason midlife is the right time to measure it.
